Your CDM Patients Get 5 Sessions. Then They Disappear.
This playbook shows you how to convert 40–50% of them into ongoing private patients worth $3,000–5,000 each — updated for the new GPCCMP framework.
887,000 Australians received physiotherapy under chronic disease management plans in the most recent AIHW data. Each patient receives 5 Medicare-subsidised sessions per calendar year. For 80–90% of them, Session 5 is the last time they walk into your practice.
Not because they don't need care. Because nobody asked them to stay.
This playbook gives you the system: when to have the conversation, what to say, how to price ongoing care, and the automated follow-up sequence that converts 30% of non-converters in the 21 days after their final CDM session.

The Complete Conversion System
On 1 July 2025, the GP Chronic Condition Management Plan (GPCCMP) replaced the old CDM/GPMP/TCA structure. Five key changes and what each one means for your practice — including why referrals no longer naming a specific provider means the practice patients find ONLINE captures the referral. The 5-session cap is unchanged. Transition period runs to June 2027.
Why Session 3 — not Session 5 — is the conversion window. The exact wording that frames continuation as a clinical recommendation, not a sales pitch. What to say, when to say it, and how to follow through at Sessions 4 and 5.
Pay-as-you-go (flexibility), 10-Session Pack (most popular — anchoring psychology), Monthly Membership (best value for chronic conditions). How to present them using anchoring to drive middle-option selection.
Day 1 SMS, Day 7 email, Day 21 SMS, Day 45 email, Day 90 SMS — with exact message copy for each. Day 21 is the highest-conversion touchpoint because symptoms typically return 2–3 weeks after stopping treatment.
How sending “ongoing private care recommended” in your Session 5 outcomes report builds trust with referring GPs and generates repeat referrals. Quarterly GP update email template included.
The word-for-word Session 4 conversation that reframes the financial transition. 54% of Australians hold PHI extras covering physio — most CDM patients don't realise their extras can fund continued care with minimal gap.
The Patient Journey: Where Conversion Happens
Session 3 is your conversion window. Not Session 5.
20 CDM patients/month × 45% conversion × $3,500 LTV = $31,500/month in lifetime value from patients you're already seeing.
